you need the ip address of the camera dvr and the ports that it uses. If you have a mouse attached to the dvr, you can log in and go to the network settings and it should tell you what the ip address of it is. Or you can log into your old router temporarily and see what NAT rules have been setup.

Then on the bt router, you need to login to that, go to NAT settings, and create rules for any ips on the outside, on the port your dvr uses (there may be more than one) to the ip of the dvr.

I'll try to keep it simple.
Normally all outside access to your home network is blocked by the firewall in the router/hub. Good thing if you don't want nigerians stealing your data, not so much if you're trying to access your cctv.
So you set up a virtual port in the router, by which you'll have access through the firewall to the device with the corresponding port number(s). In this case 34567 and 1111. Set these in the router and it should be working again. Can't help you with the port forwarding setup, that's router specific. Maybe Google knows

You will likely need to reconfigure a dynamic DNS service so that you can locate your cctv camera when the IP address for your house changes. It is in the advanced settings to add one of these services. Once added you should be able to access from that service

Found this PDF which should guide you though the process of adding forwarding ports. on the bt hub 5.

You will need to use your information though.

                      IP of the CCTV :192.168.1.177

TCP Ports to forward: 34567, 1111
