Martyn or anybody else who's done this...why does the instructions on Martyn's HID kit say to ground the HIDs to the car body?
Can't you use the ground from the lighting circuit?
Is there a reason for this? Does it make any difference?
The 'ground' is the car body which is connected to the battery. By connecting to the lighting one you are having to travel along a length of cable which may have detrioated. Earthing on the body will give a better electrical contact if done right.

It will make a big difference, it's the same as if trying to ground a car amp to the battery rather than the body.
By grounding to the body you allow for a bigger current flow due to the larger metal surface area of metal of the body with therefore more free electrons for conduction.
Why didn't I just follow the directions that came with the kit?!
There is a reason they tell you too ground the kit to the car body! Because it works properly that way! Jamo & Greg convinced to do it the proper way and voila!
They work just fine.
