I know what it is. I am surprised that it failed in this way. Any BTDTs?

Item 14A inside ("needle bearing" (3B/ABY (and AAN and ADU))

Any BTDTs? Or speculation on the reasons for the failure?

Original? I have seen these fail at high mileage on cars with mostly in-town driving.

So that implies that the number of clutch engage/disengage cycles would be very high and that somehow this fails the bearing. Almost seems like the rollers need lubrication where they contact the transmission shaft because otherwise, the shaft is trying to drag the rollers (needles) out of their housing on every cycle (?? )

I would think that any bearing would need lube of some sort. Maybe something like a molybdenum disulfide paste. Something that would not seep like grease, but a paste would stay put.

Looks like it just got old and the grease escaped, then the bearing disintegrated. I've always added some grease to mine when refitting the gearbox
