I would seriously consider changing to NGK if you have some of these in your engine.
Broken off, that's really bad if it's a manufacturing issue. I know there have been problems with the ceramic breaking but haven't heard of the electrode falling off!
I've been using the NGK's since putting my car back on the road and having it remapped. No problems with the BKR8EIX and they are cheaper too Not sure what you're running presently but they cope with >450 no problem.
Is there 100% info that a new dpor plug installed with a plug tool or with a magnet to lower it down in the head without hiting the fragile part got broken?
Also NEVER gapped one?
Asking this because once the plug escaped from my tool when i was lowering it in its place and hit the cylinder head.Got it out and saw it bent.Wanted to get the pliers to regap and realised will harm.Still did it for 2 times and forced a little. It broked.
Glad I changed mine before anything dropped off. Tried a few now and the NGK recommended PFR7B for the RS2 seem to be holding up though regapped to 0.6mm

Had a Brisk spark plug (no idea why they were fitted in the first place) porcelain break off the other day.
Owner was extremely lucky. Hole in the exhaust valve, but it seems the wastegate was open at that point, so it went through the wastegate. No damage to turbo or cylinder.
